Next have your patient or patient's representative complete the lending agreement. The Lending Agreement releases STLHELP and referring professionals from liability related to the use of equipment:         

   a. Adult Clients: Complete the check out form for your clients or their caregivers to complete

   b. Pediatric Clients: Parents/Caregivers: Complete the checkout form

3. Pick-Up: Once approved, equipment will be staged for pick-up at one of our warehouses or HUBs. You or a designated person can collect it at the scheduled time.
